Recurring Service and Automatic Renewal Addendum

This Addendum applies to the recurring service plan identified in Service Order [ID].

1. Continuous service. The plan begins on [Start Date] and continues until Customer or Rotuk terminates it under this Addendum. It does not silently renew into a new fixed term unless Customer affirmatively selects and accepts a separate fixed-term renewal.

2. Frequency and season. The normal frequency is [Frequency]. The active service season is [Season Rule]. Weather, growth conditions, safety, customer pause, route holidays, and legal restrictions may change individual visit dates. The PWA will display scheduled and skipped visits.

3. Recurring charge. The current charge is [$Amount] [per completed visit / month / event], plus applicable tax and approved extras. Customer will be charged using the separately authorized payment method [timing]. If the amount is variable, the basis for variation is [formula or scope rule].

4. Price and material changes. Rotuk will provide clear advance notice of a material price or term change and instructions to manage or terminate the plan before the change takes effect. A change that requires new affirmative consent will not take effect without that consent.

5. Cancellation. Customer may cancel at any time through the Rotuk account under You → Legal & plans → Cancel plan, or through [email protected] or 763-733-9111. Online cancellation will not be materially harder than online enrollment. Cancellation is effective no later than the period required by applicable law and may be scheduled for a customer-selected future date. Completed work remains payable.

6. Annual notice. At least once each calendar year while continuous service remains active, Rotuk will send a written notice stating the service terms and how to manage or terminate the plan.

7. Confirmation copy. After acceptance, Rotuk will provide a retainable confirmation containing the service, frequency, recurring price or pricing basis, minimum obligation if any, cancellation policy, and cancellation channels.
